We deliberately limit our scope so we can deliver each service to a high clinical standard. We are not a general disability provider — we are a nursing practice with a clear focus.

Service 01

Wound Care

In-home wound management for surgical wounds, chronic ulcers, pressure injuries, and complex skin integrity needs. We assess, dress, monitor progress, and escalate to your GP or specialist when the clinical picture changes.

Post-Discharge Nursing

You manage your own funding and can choose any provider. We invoice you directly and you claim through the NDIS portal. No plan manager required.

Plan-managed NDIS participants

Your plan manager handles the payment. We invoice them directly after each visit. You don't need to deal with any financial administration.

People coming home from hospital

Returning home after surgery or a medical admission is a vulnerable period. We provide scheduled nursing visits in the days and weeks following discharge to support safe recovery.

Yes. We currently accept both self-managed and plan-managed NDIS participants. We are an unregistered NDIS provider, which means we are not currently available to agency-managed (NDIA-managed) participants. If you are unsure about your funding type, call us and we will help you figure it out before any commitment is made.
We provide in-home wound management for post-surgical wounds, chronic ulcers (including leg and foot ulcers), pressure injuries, and complex skin integrity issues. This includes assessment, dressing selection and application, wound progress monitoring, and escalation to your GP when clinical indicators change. We do not provide emergency wound care — for acute bleeding or infected wounds that require urgent attention, please contact your GP or present to the nearest emergency department.
Yes. We provide in-home continence assessment, bladder and bowel management support, catheter care, and NDIS continence assessment reports. We can also liaise with Northern Health's adult continence service at Craigieburn if you are already engaged with that clinic and need home-based NDIS-funded follow-up care that the public clinic cannot provide.
Our primary service area covers Wallan, Kilmore, Craigieburn, Epping, Beveridge, Wandong, Mickleham, and Donnybrook. If you are in a nearby suburb not listed here, contact us and we will let you know honestly whether we can reach you within a reasonable time frame. We do not service metro Melbourne at this stage.

Yes. We are a two-nurse practice and we do not use subcontractors or a rotating pool of staff. You will see the same nurse — or in some cases both of us — at every scheduled visit. This is deliberate. Clinical continuity matters, particularly for wound care and continence management where progress over time needs to be observed and documented by someone who knows your baseline.
Yes, where clinically appropriate and with your consent. We document every visit and can provide written updates to your GP, specialist, or plan manager. If we observe a change in your condition that requires medical attention, we will contact your GP directly or advise you to seek care. We don't operate in clinical isolation — your whole care team should know what's happening at home.
